Awesome, you’re ready to schedule an appointment with us! Online scheduling makes booking your appointment convenient and super easy. Intake Forms + Email
Upon scheduling your first visit you will receive your New Patient Intake Forms and a welcome email with important information about your first visit. Please fill out the New Patient Intake Forms (about 7-12 short minutes) ahead of your visit, this will allow us to have a good idea of what’s going on and make your first visit run smoothly.

First Visit
Your first visit will be 1 hour and will start promptly at your scheduled time. We’ll begin with a conversation to ensure we have a clear understanding of your medical/performance history and goals. We’ll perform diagnostic tests to determine the root cause of your issue, and move into treatment to help relieve your pain right away.

Clothing
We ask that you wear clothing that allows you to move freely and have access to any injured body parts. Consider wearing shorts, sports bras, open back/neck shirt, or a short sleeve/tank top shirt. We understand this is not always possible, and will do our best to accommodate accordingly.

Moving Forward
At the conclusion of your first visit, we will make recommendations on a personalized course of care to get you out of pain and build resiliency against future injury. Follow-up appointments are 30 minutes. The majority of our patients utilize a visit-package—this allows both parties to commit to completing a full course of care, which ensures enough treatment to make long-term, lasting changes.

Insurance + Payment
We are out-of-network with all insurance providers and do not directly bill through insurance. You may be eligible for reimbursement from your insurance by providing an itemized “superbill”, which we provide following your visit. Our patients pay directly for their personalized care sessions.
